Insurance information

Our physicians participate with three insurance plans - SelectHealth, Sagamore PPO, and Blue Cross Blue Shield (Traditional only). Patients with coverage by these listed insurance carriers will be responsible for only a specified co-payment at time of service. There are many other health plans we provide services for on your behalf; we cannot possibly list them all. Please be appraised of your own particular health plan; as you know, there are many varied forms of coverage.

Insurance claims will be filed for you by our office for all services except cosmetic. You must provide us with the insurance information, so please bring your most current insurance ID card with you at the time of your visit. If you received a new card from your insurance carrier, please write "old" on the one it is replacing and carry both for a short time. Always show your newest card, even if it looks like the old one.
